Couture Optical of Carroll Gardens works with many insurance carriers and payment plans.

For major insurance carriers that we accept please see our carriers below: Medicaid, Medicare, BCBS, Aetna, Cigna, GHI, Emblem Health, UHC, Magnacare, UHC Community Plan, Eyemed, Superior Vision, Metroplus, GVS, UFT, Davis Vision, 1199.

For Low Vision Services: BCBS, UHC, Aetna, VSP, Eyemed, Empire, HF (Medicare), Fidelis (Medicare) are accepted with a $300 Low-Vision Evaluation Fee. The following Major Insurances that Fully Cover the Low-Vision Evaluation Fee are: HF (Medicaid), Fidelis (Medicaid). The Full Out-of-pocket Services fee is $900 for Low-Vision Evaluation.
